A native of New Jersey, Captain Michael Y. Tarlavsky was killed in action on 12 August 2004 in Najaf, Iraq.

Tarlavsky was commissioned as an Infantry Officer following his graduation from Rutgers University in 1996. In 2001 he graduated from the Special Forces Qualification Course at Fort Bragg, North Carolina, and was assigned to the 5th Special Forces Group, Fort Campbell, Kentucky, where he deployed in support of Operations ENDURING FREEDOM and IRAQI FREEDOM.

CPT Tarlavsky was serving as a Detachment Commander in 1st Battalion, 5th Special Forces Group at the time of his death.
